HEALTH DECLARATION AND LIABILITY WAIVER

Please read carefully before confirming your booking.

I declare that I am in good physical condition and physically able to participate in the surf trip I am booking. I confirm that I am fit to take part in this trip, that I am of legal age and responsible for my own actions.
I acknowledge the recommendation to purchase travel insurance with at least US$ 30,000 in medical coverage and coverage for adventure sports such as surfing, which is the purpose of this trip. I freely acknowledge and assume all risks, known or unknown, and accept full responsibility for my participation.
For group trips, I authorize the trip guide to take the actions reasonably necessary to arrange assistance for me if required.
I understand that serious accidents may occur during this trip. I voluntarily assume all risks involved and their consequences, which may include trauma, disability and death. The activities involved in this trip carry significant risks, including dislocations, sprains, trauma, fractures, accidents, possible permanent paralysis and death.
I also declare that I have a medical certificate authorizing me to practice surfing at an amateur level. The travel agency may request this certificate at any time. If an accident results from a pre-existing medical condition that could have been identified through such medical assessment, I release the travel agency and its direct or indirect service providers from liability in relation to that condition.
I understand that, if a medical team is made available during the trip, its role is limited to first aid, initial assistance and referral to hospitals when necessary. The team will handle only minor cases; when requested, the participant is responsible for going to hospitals and/or medical facilities. After this initial first-aid assistance, all further medical expenses are the participant's responsibility.
I understand that the Travel Agency is not responsible in any way for medical care provided to me by persons who are not part of the travel agency.
On behalf of myself, my heirs, legal representatives and close relatives, I release and discharge the Travel Agency, its staff, agents and employees from legal liability in relation to any injury, disability or death that I may suffer.
In the event of an accident, my family and I release the Travel Agency from legal liability for material or physical damages arising from that accident.
I voluntarily declare that I have understood and am aware of the entire content of this waiver, as well as my rights and obligations during the trip. I therefore release all parties from legal liability for events that may occur to me as a consequence of my participation in the trip.
